I was born on May 6th, 1992 in Dayton Ohio.

On my birth record it says i was born 5:56 AM

On my birth certificate it says i was born on 5:43

One says i'm a Taurus Ascendant the other says i'm an Aries Ascendant. How is there two different rising signs by 13 minutes and it didnt even change hours?

The Ascendant doesn't change every two hours exactly on the hour -- it depends on the latitude and longitude of where you are
If you were born 100 or 1000 miles north, south, east or west of Dayton Ohio your Ascendant would be different.

the ascendant degree moves one degree every four minutes.

Actually there are fast rising signs and slow rising signs.
Cancer is rising very slowly, whereas Aries is a fast rising sign, it only takes one hour; that may be the reason there are more people born with Cancer rising than with Aries rising in middle europe.
