He is literally the worst person I've ever meant in terms of remember things, common sense, etc and I can't figure out why based on his chart. Can anyone see what might be behind it and know of what might help him become more focused? Thanks.

Could possibly the Sun Opposing Neptune that I am seeing. Neptune tends to be the planet were our imagination becomes strong and we get distracted from reality. So with the Sun Opposing Neptune, there' a tug of war with imagination and reality. I hope I am making sense.
I am also strongly considered an airhead by everybody who meets me and I have my Sun, Mercury, Mars and Venus Conjuncting Neptune.

I agree with Pete that's the first thing that stands out and perhaps Mercury inconjunct Uranus + Neptune as well

Mercury in 12th using the whole sign and equal house system and also mercury being inconjunct Uranus and exactly conjunct SN.
