This would be my third live birth if I am (I've had one miscarriage, my first pregnancy ended at 13 weeks).

I wanted to add that 5th house Pluto transit has been happening since late 2008 & is set to continue until early 2019

(oddly, when my two kids will be 16 & 18)

Well, yes.

Using astrology knowledge of course. I am a little confused by transits that indicate pregnancy / fertility and I'd like help reading this.

You'd have to take in consideration a lot of other things(Progressions, SR, etc). I would say no with transits alone.
