Is it possible she is taking drugs? It seems to me that she is taking something that is particularly severe on her, and has the potential to give her a stroke. Yes I agree with nine, but more so urging her to say goodbye to this phase, either that or she is burdened by a past phase when she was endangering her health in this way. So kind of an awareness on her part that she needs to move on for her own wholeness.

She's still affected by that part of growing up when you dress to stand out, and you get into heavy sh*t to shock and impress others. But I'm feeling she is older that what the norm is for this phase.

I have a friend like that and she is only a year younger than me so yeah she's 45, and she still tries to get the applause from counter culture others and get attention in this way - but she's beyond trying to keep the belonging up with these wild others - she is so psychologically stuffed up from all the drugs and alcohol and her now violent personality, that she just lives a kind of dysfunctional existence.

So I would say that your friend has an immaturity or a resistance to mature thing about her, where by she is stuck in a phase or haunted by a phase that is not beneficial for her potential. Its like a call to snap out of it and leave all the cliche 'cool' drama behind, or longing for that, behind.

She's only 25 of age and doesn't take drugs or alcohol - but i understand where this is going and the rest...

I pretty much agree with Nine's quick guess and Chryseis's similar thoughts. To elaborate, it could be suggested that those departing on the train are aspects of herself, symbolizing various parts of her outer persona (costumes) as well as those people/friends who are associated with the old her. The train symbolizes the rigid, unchanging path that was her old life pattern. It could be that your friend has reached a phase in her life where there is change (fire/burning/melting/disintegrating) ... the old is being replaced by the new. While necessary and usually a good thing, these sorts of changes can certainly be emotionally painful (torn flesh/screaming).

If her other dreams are similar in nature, I would guess that they are an indication of anxiety and stress in her life at the moment due to changes she is working through.

What does she think? Does this make sense? If not, we'll try a different approach.
