Well, that is perfectly understandable lol. Also, as cliche as it may be, Cancer males who grew up being forced to conform to the standard, not very Cancer-ish, male archetype can have the nastiest "shells." Of course, deep below all of that is a very, very, VERY repressed bleeding heart. But sometimes we can be talking subterranean levels. And since his Cancer planets are the ones Neptune is squaring, that can hit even harder.
I know one guy with his Sun and Moon in Cancer, with his Saturn and Neptune opposing them, and he is downright evil to the naked eye. He grew up being pushed into various athletic clubs and teams by his mother (I'm oversimplifying a little, there were many other things he was forced to do by his well-meaning family), despite the fact he was chubby and not coordinated, pretty much the opposite of someone who would be interested in sports. Being Cancer, he had such a close relationship with his family he didn't want to let them down, and just tried to excel at sports and become the athlete son they wanted.
Fast forward to adulthood, he's still an athlete but he's completely disconnected from his sense of humanity. He has a literal hatred of other people. He's deviant and plays them against each other, he has "friends" but really, they're just hangers-on that he just uses for his personal gain. Unfortunately, this kind of calculating behavior is what gives him the competitive edge on the field. He has little regard for women as well, he treats his girlfriends like gold until the second they don't want to do what he tells them to. He doesn't hit them physically but he certainly bullies them emotionally until they either comply or leave, and the ones that leave have to face being shamed in front of everyone because he airs out all their dirty laundry, which he covertly gathered by playing the part of loving, caring boyfriend.
Sounds doubtful that there is a intuitive, sensitive guy behind all that, but there is. He just learned that his gifts with which he could do good in the world had no value at an early age, so he instead used them to do bad, repressing his natural instincts on principle.
And an interesting thing to note, this guy I know also has Mars in Leo like your guy. Interesting.
