That's a natal (birth) chart. It shows you the positions of the planets at the time of you birth. Some believe planets,their sign positions and angles they form among themselves influence our nature and personality.
It is very trying to tell *everything* about it(and especially if you don't say pretty please),since it is a very complicated and fine process.
It can mean many things. Personality,appearance,soul,aspirations,preferences,etc.
