Well it really depends on your natal. I have my Vertex conjunct Uranus in the 6th and Chiron conjunct my Ani-Vertex in the 12th (obviously )The fact that I have planets conjuncting/opposing my Vertex activates it naturally so I get a lot of action when a planet like Saturn crosses its path.
In my case, during this year's Saturn Rx, it passed over once in January, Rx'd over it again in March and will conjunct it once more in September.
My experiences were really painful and I would not wish them on anyone, plus I don;t want to freak you out. Having said that, the nature of my chart and the relationship I had with my ex ( in our synastry, my Saturn conjuncts his Vertex) made for a very Saturn/Vertex/ fated ending between us.
It really depends on your chart I think. Asteroid Karma works the same way for me. I have it conunct natal Pluto exactly and on the day I met my ex T. Karma was conjunct my natal Karma. So, my point is that your natal chart will in part determine the strength/manifestation of the transit.
I would suspect that regardless you will notice something about the conjunction when it is Saturn dropping by, but without seeing the rest of your chart or knowing how Vertex and/or Saturn affect your synastry or even Solar Returns, it's hard to say just how it will manifest.
