What would signal major health improvement? Transits throught the 6th house aspecting which natal planets?
How about Tsaturn conjuncting NUranus?
What others would be involved?
Chiron? Pluto? Jupiter?

And also the other way around: what aspects would decline health? Would that also happen in the 6th house or would another house be prominent?

Umm.. good question.
I think Jupiter in good aspect to 6th house planets or in the 6th, would improve health. The ruler of the 6th house could play a role, also.

The malefics could create some trouble: Saturn, Mars, and even Neptune, Uranus and Pluto.

Personally I had health issues when transiting Neptune was squaring my natal 6th house planets (and it was very Neptunian: psychological problems mainly, but which ultimately lead to physical illness, too).

Overall I think Venus/Jupiter in good aspect to ASC, 6th house and their rulers could point to health improvement.

And outer planets with Saturn/Mars, especially afflicted, could point to health problems.

I had major health problems as a child, during that time my health really gave my mother hell. This was when Tr. Saturn was in my 12th house, natal Saturn widely conjunct Tr. Neptune and Tr. Chiron in my 6th conjunct my Vertex.
Health improvement once Tr. Saturn was in my 1st house.

Checked for aspects and one I found was Tjupiter and Tvenus conjunct Nchiron (for major improvement).
