I know Neptune in the 10th house causes problems with focusing on a career but what does it do for reputation or how others see you? A lot of celebrities have this placement.

It can be tricky in that people don't see the real you and can project a lot onto you, a bit like when Neptune is on the ASC.

You can attract a lot of admiration / fascination but also paranoia and suspicion, and there can be false rumors about you or some confusion as to who you are exactly.

And yes confusion with the life purpose is likely.

I just see this placement in many successful celebrities' natal charts and most of them are well-liked, even revered.

I feel people gossip about me to a high degree, even though I don't do anything gossip-worthy. I feel people want to dig into my life...it is very odd.

Neptune can cause problems wherever he goes. He just doesn't fit into the earthly plain that well, this is what you have to watch out for. It's not that Neptune is bad, just the human species doesn't respond to Neptunians that well.
I've learned this from my own afflicted Neptune.
