It is said that Saturn transiting over the moon is the most difficult time. Is there a difference between when it makes it's first forward pass over the moon and when it retrogrades back over it? Or do both movements create the same issues in a person's life?

It is the same combination of issues. When the direct motion takes Saturn away from natal Moon's position, there can be relief from the seriousness/heaviness. Saturn-Rx back to natal Moon's position brings back matters for further processing that we may not have wanted to look at.

This isn't to mean that it is all heavy stuff or 'issues' but that we get a break from the time of being more internal, feeling less social. Then can get back to getting more in touch with ourselves. Occasionally we all need this influence of Saturn to rein in the expansive or social dynamic some may feel in ongoing response to external social / familial stimuli, but we need the internal awareness too.

Moon in Sag can have great difficulty with that.
