So through derived houses you would start at the 4th house, since this is typically your maternal side. Maternal great grandmother is the mother of your mother's mother.
Therefore:
the 4th house from the 4th house (the 8th house): Your maternal grandmother
the 4th house from the 8th house (the 12th house): your maternal great grandmother
This is assuming your own 4th house represents your mother. If the 10th house more represents your mother then you would need to use that instead, same concept.
To put it like this: you would make your own 4th house the imaginary 1st house of your mother. Then, from there, you would go to her imaginary 4th house: this is her mother. So her imaginary 4th house is now her mother's imaginary 1st house. From there, you would go to the mother's mother's 4th house: this is your great grandmother.
