Do you mean Mercury and the Sun? Because they never get TOO far apart...We put an imaginary circle around the Earth, and divide it into twelve parts. We name the parts for the Stars they contain. This is the Zodiac, the twelve Signs.
The Planets revolve around the Sun. They are like an ever-moving ten-geared clock. When they form angles to one another, we call them aspects. They are Triangles, Squares, Oppositions, etc.
When you say Venus (Planet) is in Pisces (Sign) you just point to where (in the sky) that Planet was visible. A Natal chart is just a still life picture (sort of a "You Are Here") of the Heavens at the moment you first took a Breath.
A Transit does the same thing at any given moment to see if the current placement affects the ones you were born with.
Venus in Pisces is an exalted placement, Venus is very happy when riding in that place in the Sky, it agrees with her. It's more like liverwurst when she sits in Virgo.
I hope this helps?
