Lets do this with some other signs on the cusp.Say you have Aries on the cusp of the 6th and mars in the 1st. Would you be prone to fevers and maybe accidents.

What if we have Gemini on the cups and Merc in the first? Would you illness of the parts of the body mercury rules. I think it is the hands.

Could we do this with every sign and the ruler( and the parts of the body it rules)

With gemini, could you have mental illness cuz Gemini is the mind?

I have to respond to this thread just because I've got this very thing in my chart: Pisces on the 6th house cusp with Neptune in the 1st. However, I've also got my chart ruler, Venus, in the 6th so that might modify my experience.

My "health issues" tend to be overlooked. My hearing loss (about 50% in both ears) wasn't noticed by anyone until I was over 30 because I adapted to it so well. The difference in leg length that started eventually to cause hip and back problems were never actually diagnosed by a doctor, but I became aware of them and have compensated for them by using an orthotic in one shoe but not the other (and as a result the hip problems improved and the back problems -- how shall I say this? -- my self-diagnosis/solution is transforming my body for the better). My foot problems were never officially diagnosed but I became aware of those when my oldest sister and then my youngest sister developed plantar facitis. I now wrap my feet when I dance and at 52 still dance in pointe shoes. I'm sure some of this is through the benific influence of having Venus in my 6th house and in mutual reception to that 1st house Neptune, but I'd say you'd have nailed that one otherwise. If it was just that one aspect under consideration and not the chart as a whole, I'd say you'd gotten it spot on!

What is hard for me is the various possibilities for the same House Ruler's placements. I think this is why I got so confused.

Taurus (Venus): slow, generally peaceful, maybe due to overindulgence, throat.

Gemini (Mercury): Possible multiple causes, mental deterioration possible, lungs and breathing.

Cancer (Moon): At home, overeating may be a factor, stomach.

Leo (Sun): probable cardiac problems, some drama.

Virgo (Hygeia): Often a quiet demise in modest circumstances, not dramatic, digestive system.

Libra (Venus): Not difficult, maybe related to kidney function.

Scorpio (Pluto): Possibly confined or violent, alcohol.

Sagittarius (Jupiter): Abroad or away from home, possibly overindulgence.

Capricorn (Saturn): Slow degenerative demise probably in old age.

Aquarius (Uranus): Sudden, possibly unusual demise, rare diseases, blood problems.

Pisces (Neptune): Confused; cause of death obscure or uncertain, alcohol, poison, drugs, mistake, suicide.

OK--if we take this list and put these signs on the 6th house cusp and the ruler in the 1st--what would we have. Do you want to speculate, for purposes of learning?

If we have Cancer on the 6th house cusp and the moon in the 1st, could be have these possibilities? Breast issues, stomach issues, stomach gets stressed easily etc

To add to this. If we had Jupiter conj the Moon in the 1st house, might the person overeat or have very large breasts?

I think the smaller planet transits, especially mars as you said would cause a minor accident such as tripping over and cutting yourself or something like that
Also I think aspects could count here like you said with Saturn conj Moon, you could have problems in your teenage years with developing breasts, also Moon rules the stomach so you may have a small apetite caused by your stomach size or it could indicate gastric band surgery or something similar via transit
Same if you have say Saggitarius in the Sixth and Jupiter in first, it can indicate issues with obesity and say a square to Venus which rules hormones could mean that this was caused by hormones in the body, or it could also mean a problem with binge eating.

This website is quite interesting, it is vedic but I think we could adapt it to western astrology to see what's happening

I know for me, ruler of sixth being Mars in the Tenth house (in this case ruling the knees), it'll mean that I may have an issue with my knee joints, this to me could say althritis could be a problem for me, and it does actually run in the family
