Could be, who we go after certainly is linked to how we see ourselves, though it's not always matching.

To me Saturn builds so much that it can constipate action,like those building plans spending years in drawers before the first brick is finally laid.
In the 1st house it could mean someone who is but fails to actualise who they are through their actions, someone who's identity never comes off.
Instead of being the loser who insults girls on the street, 1st house Saturn would be the boy who's dying to address the situation but loses so much time to think about what he'll say, or design his move that when he finally speaks about it, it's no longer relevant or the situation has crystallised and his position needs to be revisited.
I would say that they exist in ways that are isolating and that this can cause themselves to doubt every portion of ourselves that is defined by others, because for them, the encounter is delayed, awkward...
A lack of esteem could also come from projection, because Saturn looks at a perfectly mastered end in sight, maybe he does presume of his own self, maybe he judges himself based on what he aspires to ultimately be which is crushing for a young soul and still so for an older one that has put itself through that treatment all its life.
But as cancer opposes Capricorn, maybe the position of the moon and its strength can help soften a Saturn placement that would be heavy on something so fragile as the person‘s ego?
Another way to look at it would simply take into account Saturn's speed and interpret it as individuals whose 1st house takes longer to express itself, not because of any difficulty delaying the action, just as a natural pace. In our society it would be isolating enough, but in itself, there is nothing wrong with taking time 😊
