I have noticed Neptune in the 12th house to cause variations of personality disorders. It seems the closer it is to the ascendant, the more likely it is to manifest this way as well. Also if in aspect to Mercury I've noticed it can cause some self deception, or cause someone to be very unaware of themselves and their own behavior - also can be true if in aspect to the sun. But this is definitely heightened if Neptune or Mercury are in aspect and one or both of them are in the 12th house.
Of course I feel it is necessary to say though for anyone who may read this while searching these keywords, just because you have any of these placements does not mean it is written in blood that it will manifest this way for you too. Every placement has a scale, and can manifest in an aptitude of ways!
